Admissions RN (Oasis)-Afternoon/Evenings

Description:

The Admission Nurse is a Registered Nurse who provides direct and indirect patient care services according to Advanced Home Care's policies and procedures.  The primary responsibility of this nurse is to complete a comprehensive assessment of the patient, the environment, and the support system.

 The Admission Nurse will initiate services and complete a plan of treatment and necessary forms as needed to implement patient care.  This nurse will not be a primary caregiver but may be needed to make patient visits according to patient census.

Duties:

  •·        Assesses the physical, mental and emotional components of patients to initiate a comprehensive plan of care.

•·        Assesses the patient's environment with particular attention to safety to ensure the environment is suitable for the delivery of safe and effective care.

•·        Assesses the patient's support system and utilization of available community resources to ensure optimum level of care.

•·        Identifies nursing diagnoses/patient care problems and interventions to ensure that patient's problems are comprehensively addressed.

•·        Initiates the plan of treatment including other disciplines as needed.

•·        Demonstrates teaching skills and continued competence in performing therapeutic interventions for all levels of complexity in patient care.

•·        Recognizes changes in patient's status and reports appropriately.

•·        Uses critical thinking skills to assess patient's condition and determine appropriate interventions.

Qualifications:

Requires a degree in nursing from an accredited or approved school.  At least one year of medical/surgical nursing within the last five years and preferably one year of community health nursing.  Must have a current registered nursing license for the state of North Carolina and reliable transportation.

MUST HAVE HOME HEALTH/OASIS experience to be considered.  Otherwise, apply for our RN position.

We have several openings due to growth! This position requires Admission visits in the Greensboro or  Winston Salem area but must be willing to travel and service the entire Triad area as needed.  Hours of work are 3pm-11pm M-F, no weekend rotation or we can possibly work a flexible schedule with 4/10hr days.
