Description:

Provides general nursing care and education to patients and patient care partners within the Home Modality Department and/or patient's home.

Qualifications:

Must have current license, excellent communication/interpersonal skills, the ability to perform independent nursing with little direct supervision, strong organizational skills, knowledge of teaching/learning concepts and enthusiasm and motivation to inspire patients and their care partners are necessary. Dialysis experience preferred.
