Social Worker • Participates in the development of the plan of care for each hospice patient and provides direct social work services. • Duties include o Assessing the patient/family psychosocial response to terminal illness, o Participates in quality improvement programs, o Assists patient/family and agency staff in utilizing community resources, o Explores and evaluates financial situation and identifies appropriate resources as needed, documents all interventions accurately, completely, and on a timely basis. o Assists team members in understanding the significant social and emotional factors related to care of a terminally ill patient and family. • Three years with adult clients required. • Must possess current professional license and a driver’s license.
