We recognize that a person is more than a physical body and we provide skilled professionals and volunteers to assist with not only the physical needs, but emotional and spiritual needs as well. We also recognize the family unit, and support is offered to family caregivers that includes counseling and education.

A registered nurse is always on call, 24 hours, to answer questions and to provide assistance and support when needed. We will provide care to every eligible patient in our service area, regardless of medical conditions, lifestyle, religious, cultural or ethnic background. We believe all patients who are eligible for hospice should receive hospice. After all this is the time When Life Matters Most....

Duties:

Social Worker

• Participates in the development of the plan of care for each hospice patient and provides direct social work services.

• Duties include

o Assessing the patient/family psychosocial response to terminal illness,

o Participates in quality improvement programs,

o Assists patient/family and agency staff in utilizing community resources,

o Explores and evaluates financial situation and identifies appropriate resources as needed, documents all interventions accurately, completely, and on a timely basis.

o Assists team members in understanding the significant social and emotional factors related to care of a terminally ill patient and family.

• Three years with adult clients required.

• Must possess current professional license and a driver’s license.
