Duties:

-Assist in developing an environment that maximizes each persons choice and ability in all their daily routines and living skills consistently on a daily basis.
-Provide support and training to individuals as identified in IPP consistently.
-Serve as "Primary" staff to the residents and complete all necessary responsibilities, such as Safety Drills, LAF reviews, Consumer meetings, attendance forms, etc
-Ensure individuals physical and mental needs are being cared for by assisting the individuals in arranging necessary appointments. Assisting with transportation as needed and assuring the appropriate medical documentation has been completed.
- Assist the individuals in planning and implementing individual or small group social/recreational activities.
- Provide transportation when necessary for shopping trips, banking, business, etc
- Assure accurate documentation and filing according to agency and program policies. Daily documentation is required to be completed prior to leaving your shift.
- Regular and reliable attendance is required.
-Promote positive communication and working relationships with all team members and community
- Attend monthly in-services.
- Attend weekly staff meetings. If you cannot attend, notify the RPI as to why you are unable to attend. Read the team meeting notes and get the necessary clarification from the RPI, Lead or Assistant Lead.
- Support Hope Haven's philosophy, vision, and mission statements.
- Follows all safety procedures and promotes safety measures among team and clients.

Qualifications:

1. BA degree or experience in Human Services Field preferred or minimum of High School graduate or GED;.
2. Ability to work with people, providing support in all aspects of daily living as directed by each person's individual plan.
3. Good written and verbal communication skills.
4. Ability to exercise sound judgment in making decisions.
5. Ability to complete/pass required medication administration course.
6. Ability to complete R.A. course and/or required orientation training and all other agency training requirements.
7. Possess a valid drivers license and ability to acquire an Iowa Type "D" license with endorsement 3 when required.
8. Up-to-date physical.
9. Clearance from a criminal background check and abuse reporting check at time of hire.
10. Regular and reliable attendance is required.
