Description: | Provide case management to clients in the Beyond Pathways program
Beyond Pathways is Urban Ministry's privately funded "housing first" program which rapidly rehouses homeless families from our emergency family homeless shelter.
| Duties: | • Provide case management to Beyond Pathways’ clients (referrals, advocacy, consultations, budgeting, etc.)
• Make regular home visits to clients
• Coordinate services with the Welcome Home Teams
• Oversee GUM payment of clients monthly rent and utility bills
• Maintain proper case notes and documentation on all clients
| Qualifications: | • Bachelors’ degree in Social Work
• Minimum 2 years experience in human services
• Strong skills in organization and attention to details, excellent written and verbal communications, strong customer service skills, ability to work with a broad spectrum of people, knowledge of community resources
• Certified or licensed social worker preferred |
