Description: |
The Registered Nurse filling this position will work as a part of our call center team. You will conduct initial member assessments; triage incoming telephone calls; respond to questions regarding medications and treatment instruction; record and report member’s condition; provide education and teaching; and answer questions regarding diagnostic testing and treatment.
At SynCare we believe that healthy outcomes occur by extending the physician-patient partnership into the community, by providing one-to-one interactions, health education, motivational interviewing and behavior modification. Our programs are designed to increase the use of appropriate services by providing our members relevant health education and, where appropriate, encouragement to modify behavior.

Duties: |
You will utilize an appropriate amount of telephonic coaching services and home visits to help individuals in these at risk populations. You will be improving the quality of care, increasing use of appropriate services, providing appropriate health education and encouraging behavior modification.
Our services target high users of health care services and those that are high risk and/or with chronic diseases. We focus on prevention, education and behavioral change. You will have in house Social Workers available to help where needed.

Qualifications: |
• RN - licensed in the state of Indiana;
• BSN Preferred;
• 2 or more years of clinical experience is preferred:
• Bilingual in English and Spanish is highly desirable;
