Clinical Coordinator

Description:

Successful candidate will have tremendous opportunity for professional growth and leadership within a highly motivated, highly innovative clinical team. The primary role of the Kerr Health clinical pharmacist is to provide quality, evidence-based patient care, and serve as primary preceptor/mentor for residents and PharmD candidates.

Duties:

• Monitoring and collaborative management of patients with asthma, diabetes, dyslipidemia, hypertension, and other chronic diseases
• Administration of influenza, pneumococcal, and other adult vaccinations
• Provision of MTM services for a variety of state and national payers
• Marketing to payers and other healthcare providers
• Delivery of corporate wellness programs including health screenings, analytic risk assessments, and 1:1 employee education regarding modifiable risk factors
• Maintenance of appropriate documentation and billing functions
• Clinical and business management of practice site; supervision of administrative support staff
• Grant-writing; participation in practice-based research

Qualifications:

Education

Doctor of Pharmacy
Community or Ambulatory Care Pharmacy Residency


Experience

Minimum 3 years practice experience, preferred


Degrees, Licensure, and or Certification

North Carolina Pharmacy Licensure
Certified Diabetes Educator, preferred


Knowledge, Skills, and Abilities

• Strong clinical skills with knowledge and application of appropriate standards of care
• Excellent oral and written communication skills
• Strong organizational and time management skills allowing concurrent work on multiple diverse projects, with ability to effectively prioritize and maximize efficiency
• Ability and willingness to work independently and as contributing member of dynamic clinical team
