Duties:

Job Responsibilities:

• Performs routine home visits, death visits, respite/continuous care cases, and/or personal care visits, as assigned.

• Updates and evaluates care by performing patient assessments and considering emotional, social, psychological, physical and spiritual care needs.

• Observes and reports patient changes, reactions, or abnormal symptoms to RN or immediate supervisor.

• Provides patient/family education through instruction (verbal, demonstrations, etc.) and feedback.

• Follows RN’s direction of Hospice death procedure when attending a death.

• Assists with more complex patient care and treatment procedures

• Performs other relatively non-complex patient care treatments

• Demonstrates courteous behavior, meets patients/family’s needs, responds promptly and offers assistance to meet patient, family and co-workers requests.

Qualifications:

Education:

Required:

Must be a graduate of an accredited School of Practical Nursing. Experience:

o Desired:

Hospice, Home Health, Private Duty, Oncology or Medical/Surgery experience.

Licenses:

o Current license to practice nursing and current driver’s license.

CPR Certification
