Description:

Child and Family Connections, Inc. is seeking qualified applicants for the following vacant position.

the Permanency Specialist coordinates and facilitates all staffings related to dependency proceedings, including, but not limited to transfer staffings, permanency staffings and other critical juncture staffings, with the goal of expediting permanency for children in the dependency system.

Duties:

Schedule and facilitate staffings in accordance with CFC policy and Florida Statute, or as dictated by case needs.

Responsible for guiding process towards permanency to include case plan recommendations and follow-up.

Ensures transfer of information between Child Protective Investigations staff and Dependency Case Management.

Work collaboratively with case management staff, CFC Operations and Clinical Services staff to ensure case needs are addressed and met.

Documents all case related activities in the Florida Safe Families Network.

Participate in the Quality Improvement Team.

Ensure appropriate services have been identified and provided for all case participants.

Work with the Case Management Coordinator to respond to and resolve stakeholder inquiries, complaints and grievances.

Identify barriers to timely permanency, performance needs or practice trends and communicate needs within Program Operations and across departments as needed.

Other duties as assigned by management.

Qualifications:

Required:

Bachelors Degree in social work or other human services related field.

Two years child welfare experience

Active Certified Child Welfare Professional

Proficiency in word processing, spreadsheet software and database applications.

Preferred:
Prior supervisory experience
